27例肝脏损伤治疗方法探讨

目的探讨肝脏外伤的治疗方法及其疗效。方法对27例肝外伤患者的临床处治资料进行回顾性分析。结果最容易受伤的部位是肝右叶膈面,占51.9%;AAST分级以Ⅰ～Ⅲ最为多见,占66.7%,Ⅲ级以上33.3%;6例（22.2%）采用保守治疗;21例（77.8%）经手术治疗;25例（92.6%）治愈,2例（7.4%）死亡。结论肝脏损伤应根据损伤的部位、损伤程度、患者血液动力学以及合并伤情况选择治疗方法。治疗过程中或术后要密切监护,防止并发症,降低死亡率。

Treatment of 27 cases of liver injury

Objective To investigate the treatment of liver trauma.Methods Clinical data of 27 cases of liver injury were retrospectively analyzed.Results The most vulnerable part of liver was the right lobe diaphragm,accounting for 51.9%;AAST grade Ⅰ~Ⅲ was common,accounting for 66.7%,proportions of Ⅲ and above were 33.3%;6 cases（22.2%） underwent conservative treatment;21 cases（77.8%） with surgery.As a results,25 cases（92.6%） were cured,2 cases（7.4%） died.Conclusion The treatment method of liver injury should be based on the site of injury,injury severity,hemodynamics,and associated injuries.The treatment should be closely monitored to prevent complications and reduce mortality.
