| 摘 要： | 目的评价多层螺旋CT（multi-slice spiral computer tomography,MSCT）在结肠癌术前分期中的准确性及诊断价值。方法前瞻性纳入2010年1月-2010年12月于我院住院的结肠癌患者120例,分析CT影像在确定肿瘤侵犯范围、淋巴结转移及远处转移方面的特点,并与病理结果进行对比。同时比较MSCT和肠镜诊断结肠癌部位的差异,分析其肠梗阻和非肠梗阻对MSCT术前分期的影响。结果 MSCT对结肠癌T分期、N分期、M分期和TNM分期的准确度分别为80.8%、64.6%、100%和70.8%。MSCT和肠镜诊断结肠癌部位的准确度为88.3%和81.6%,差异有统计学意义（P〈0.001）。完全性肠梗阻、不完全性肠梗阻和非肠梗阻患者术前CT-TNM分期的准确度分别为89.7%、86.7%和87.0%,三者差异没有统计学意义（P〉0.05）。结论 MSCT能较为准确的诊断结肠癌解剖学部位和评估术前分期,但在评估淋巴结转移方面还有待提高,此外其术前分期的准确性可能会受到肠梗阻的影响。
