营养支持对撤离呼吸机的临床意义

目的探讨营养支持对ICU内行机械通气的COPD患者撤离呼吸机的意义。方法将40例COPD致呼吸衰竭行机械通气的危重患者随机分为治疗组（A组）和对照组（B组）各20例。在机械通气到撤机期间A组给予营养支持治疗,B组未行营养支持治疗,比较两组患者上机第5天的白蛋白和淋巴细胞数,一次性脱机成功率、一次性脱机成功时间,一次性脱机成功后30分钟与脱机前相比,患者心率和呼吸增加百分率。结果与B组相比,A组患者上机第5天的白蛋白和淋巴细胞数均明显高于B组（P〈0.01）,A组一次性脱机成功率高于B组（P〈0.05）,一次性脱机成功时间明显短于B组（P〈0.01）,A组一次性脱机成功后30分钟,心率和呼吸增加百分率均明显低于B组（P〈0.01）。结论对行机械通气的COPD患者,营养支持有助于增强呼吸肌的质量和功能,有利于早期脱机。
