普通器械在单孔腹腔镜手术中的应用体会

目的总结使用普通腹腔镜器械开展单孔腹腔镜手术的临床经验。方法回顾分析遂宁市中心医院肝胆外科应用原有普通腹腔镜器械进行的140例腹腔镜胆囊切除,18例腹腔镜胆囊切开取石,2例腹腔镜脾切除患者的临床资料。结果本组患者中应用该方式完成127例胆囊切除,18例腹腔镜胆囊切开取石,2例腹腔镜脾切除。平均手术时间27～135min（68±0.46min）,平均手术出血量3～300ml（16.9±0.10ml）,术后平均住院3.3d（3～7d）。患者术后切口愈合良好,瘢痕隐蔽。随访1～8个月,无漏胆、胆管损伤、切口疝等并发症。结论使用原有的普通腹腔镜器械就能够达到单孔腹腔镜的手术效果,并且简单易行,适合基层医院推广。

The experience of common devices in single incision laparoscopic surgery

Objective To generalize the experience of using common devices in single incision laparoscopic surgery.Methods The clinical data of 140 cases of laparoscopic cholecystectomy were analyzed.Results We completed 127 cases of laparoscopic cholecystectomy,18 cases of laparoscopic gallbladder incision stone-free,2 cases of laparoscopic splenectomy.The mean operating time was 27～135min（68±0.46min）.The mean blood loss was 3～300ml（16.9±0.10ml）.The mean postoperative hospital stay was 3.3 days（range,3-7d）.All the incisions were healed well and the surgical scar was satisfied and virtually concealed within the umbilicus.No postoperative complications were observed at follow-up 1～8 months.Conclusion This method using the original laparoscopic devices can reach the effect of single incision surgery,Simple and easy,Suitable for basic-level hospitals.
