超声造影检查在诊断肝脾外伤中的价值

目的探讨超声造影检查技术在诊断肝脾外伤中的价值。方法回顾性分析38例肝脾外伤患者的常规超声结果和超声造影资料,总结肝脾破裂的超声造影图像特点,与常规超声结果比较,并计算超声造影在诊断肝脾破裂中的敏感度和特异度。结果手术或CT检查证实38例患者为肝脾外伤,超声造影检查诊断正确者35例,超声造影诊断肝脾外伤的敏感度为92.1%,特异度为67.9%,显著高于常规超声的60.5%和32.1%,差异有统计学意义(P<0.05)。超声造影提示肝脾破裂口共82处,手术所见为97处,准确率为84.5%。CEUS分级的准确率为89.5%(34/38)。结论超声造影检查的诊断及分级准确率都较高,可较准确评估肝脾外伤的程度,可作为肝脾外伤的首选影像诊断手段,值得在基层医院推广使用。

Value of contrast-enhanced ultrasound in diagnosis of liver and spleen trauma

Objective To investigate the value of contrast-enhanced ultrasound in diagnosis of liver and spleen trauma.Methods Retrospectively analysed the results of conventional ultrasonic and CEUS data from 38 cases of liver and spleen trauma patients.Summarize the characterirstics of the liver and spleen trauma CEUS performance compared with conventional ultrasonic results.Calculate the sensitivity and specificity of CEUS in the diagnosis of liver and spleen trauma.Results The results of surgery or CT confirmed 38 cases of patients with liver and spleen trauma,while CEUS correctly diagnosed 35 cases.In diagnosis of liver and spleen trauma,the sensitivity and specificity of CEUS were 92.1% and 67.9%,both significantly higher than conventional ultrasonic’s(sensitivity=60.5%,specificity=32.1%)(P < 0.05).Ultrasound imaging prompted 82 liver and spleen rupture mouthes,while surgical finding was 97,and the acurracy was 84.5%.Besides,the acurracy of CEUS classification was 89.5%(34/38 cases).Conclusion Due to the high acurracy of diagnosis and classification,CEUS can assess the extent of the liver and spleen trauma.CEUS could be used as the preffered means of diagnostic imaging of the liver and spleen trauma,and it is worth to be promoted among basic hospitals.
