| 摘 要： | 目的比较自体角膜缘干细胞移植、羊膜移植和单纯翼状胬肉切除三种手术方式治疗翼状胬肉的治愈及复发情况。设计回顾性病例系列。研究对象自2007年8月至2010年8月在解放军第二炮兵总医院眼科应用上述三种手术方法治疗并有随访记录的翼状胬肉患者193例(217眼)。方法根据手术方法的不同,将193例(217眼)翼状胬肉患者分为3组:角膜缘干细胞移植组(A组)73例81眼;羊膜移植组(B组)54例63眼;单纯翼状胬肉切除组(C组)66例73眼。术后平均随访1年(12.2±2.3)月,观察并比较三组患者的治疗效果、复发率和并发症情况。主要指标复发率和并发症。结果随访期间,A组复发4眼,复发率为4.94%;B组复发10眼,复发率为15.9%;C组复发23眼,复发率为31.5%(A与B组、A与C组、B与C组相比,χ2值分别为4.828、18.746、4.498,P值分别为0.028、0.000、0.034)。随访期内,A组未发生睑球粘连,B组有1眼(1.59%)出现睑球粘连,C组有8眼(10.96%)出现睑球粘连。结论与单纯翼状胬肉切除术和羊膜移植术相比,自体角膜缘干细胞移植治疗翼状胬肉并发症少、复发率低,是治疗翼状胬肉较好的手法方法。
