晚期肺癌全肺切除44例报告

目的:讨论晚期肺癌行全肺切除术后的高并发症及死亡率的主要原因,提出手术指征的掌握和围手术期的处理要点.方法:对44例Ⅲ、Ⅳ期肺癌行全肺切除病人的临床病理资料进行分析.结果:42例痊愈出院,住院死亡2例(4.54%),并发症发生率15.91%,随访1、3和5年生存率分别为90.91%、40.91%和20.45%.结论:晚期肺癌全肺切除术后的并发症及死亡原因主要是全肺切除后严重影响呼吸循环引起急性呼吸衰竭和心力衰竭导致死亡.只要围术期处理适当,晚期肺癌行全肺切除是可行的,再加放疗、化疗等辅助治疗,预后相对较好.

Clincal analysis in patients with advanced lung cancer undergone pneumonectomy

Objective:To ascertain the cause of higher morbidity and mortality in the patients with advanced lung cancer un-dergone Pneumonectomy. Sugical indications and peri - operative meanagement were mentioned. Methods: Clinicopathologic date of 44 patients with advanced lung cancer undergone pneumonectomy were analyzed. Results: 42 Cases were discharged when they were fully recovered. the operative mortality was 4.55% (n = 2) and morbidity was 15.91% (n= 7) . The survi val rates were followed up: one year survival rate 90.91 % , three year 40.91 % , and Five year 20.45 % . Conclusion: The causes of higher morta. lity and morbidity in these patients with advanced lung cancer were mainly acute lung and heart failured becasue of undergone pneumonecto-my. With appropriate peri- operative management, pneumonectomy is possibility in the petients with advanced lung cancer and long time survival rate may be increase.
