食管癌术后断端癌组织存留的多因素分析与防治

目的:分析食管癌术后断端癌组织存留的影响因素,提出正确的防治方案.方法:78例食管断端癌存留者与同期手术的778例无断端癌存留患者进行了多因素分析,并对71例癌存留者进行了随访.结果:食管断端癌存留与肿瘤的病理形态、浸润深度、分化程度和淋巴结转移与否有非常显著相关关系(P<0.01～0.05),而与肿瘤的生长部位和病变长度无相关关系(P>0.05),随访发现断端癌存留患者经过术后放疗,3、5年生存率显著提高.结论:根据食管断端癌存留影响因素的存在,选择广泛切除食管,可减少断端癌存留的发生,对断端癌存留患者,采取积极的术后放疗是一种有效的补救措施.

Multivariate analysis and prevention of residual tumor after operation in patients with esophageal carcinoma

Objective: To analyses influencing factors of residual tumor of the cutting margin in patients with esophageal cancer, Prensent Corrective methods of Prevention and treatment. Method:From Dec 1983 to Jun 1996,856 cases with esophageal carcinoma were studied, and divided two cases(78/856) with residual tumor were group one and the other(778/856) were group two.Cas-es(71/78) with residual tumor were followed-up. Result: It was markedly associated residual tumor with the pathologic type, the infiltration depth, the lymphatic matasasis and the degree of cell differentiation. It was no relationship between residual tumor at the resection magin and cancer length and cancer position. Follow-up of 71 Cases with residual tumor showed that 3, 5 year survival rates of radiative treatment on 32 patients wad higher than 39 cases without radiation treatment( P < 0.05 ) . Conclusion: The extensive operation should be carried out in patients with esophageal cancer who had some risk factors of residual tumor of the cutting margin. The radiative treatment on postoperative patients with residual tumor could increase long-term survival rates.
