腮腺恶性肿瘤60例临床分析

目的　对 6 0例手术和手术后放疗的腮腺恶性肿瘤进行回顾。方法　 6 0例腮腺恶性肿瘤中单纯手术治疗2 5例 ,手术加术后放疗 35例。结果　腮腺恶性肿瘤的 5年、10年生存率为 75 %和 5 1.43%。单纯手术分别为 72 %和 36 .36 % ;手术加术后放疗分别为 77.14 %和 5 8.33% ,两种方法 10年生存率对比有显著差异 (P <0 .0 5 )。有无颈淋巴结转移5年生存率分别为 41.6 7% ,84.84% ,有显著性差异 (P <0 .0 5 )。低度恶性肿瘤 5年、10年生存率为 90 %和 6 4% ,高度恶性者分别为 45 %和 2 0 % ,有极显著性差异 (P <0 .0 1)。结论　首次术式选择合理 ,术后放疗是减少局部复发的重要因素 ,颈淋巴结转移和病理分化差的晚期癌是影响预后的关键因素。

Clinical analysis on 60 cases of parotid gland malignant tumors

Objective To retrospectively study the clinical data of 60 patients with parotid gland malignant tumors who received surgery or combined it with radiotherapy.Methods Twenty five patients were treated by surgery alone,and 35 by combined radiotherapy with surgery.Results The total survival rate of 5- and 10-year was 75.0% and 51.43% respectively,and 72.0% and 36.36% in group of surgery alone, 77.14% and 58.33% in combination group respectively.It showed that there was a significant difference between the two groups in survival rate of 10-year (P<0.05).In addition,the survival rate of 5-year was 41.67% in group of lymphatic metastasis,84.84% in group of no metastasis.It also showed significant difference (P<0.05).The survival rate of 5- and 10-year was 90% and 64% in group of well differentiated tumors,45% and 20% in group of poor differentiated tumors.Conclusion The reasonable operation combined with radiotherapy after surgery is important factors to reduce the rate of recurrence.The lymph node metastasis,pathological types and differentiation play an important role in the prognosis of parotid gland malignant tumors.
