左卡尼汀治疗2型糖尿病合并冠心病的临床观察

目的：观察左卡尼汀对2型糖尿病合并冠心病患者心肌缺血症状，血糖、血脂、游离脂肪酸的影响。方法：62例2型糖尿病合并冠心病患者被随机分为左卡尼汀组及常规治疗对照组（对照组）。在常规治疗基础上左卡尼汀组加用左卡尼汀治疗，观察治疗前后心肌缺血症状，检查超声心动图及测定治疗前后血糖、血脂、游离脂肪酸浓度，并与对照组比较，分析其治疗效果。结果：左卡尼汀组与对照组相比，对心肌缺血症状有明显改善，胸闷、胸痛、乏力缓解率显著下降（P〈0．05）。左卡尼汀组血糖、血脂、游离脂肪酸水平均显著下降（P均〈0．05）；与对照组比较，血脂、游离脂肪酸水平显著下降（P〈0．05），且无严重副作用。结论：左卡尼汀治疗2型糖尿病合并冠心病患者可改善患者心肌缺血，同时降低血脂、游离脂肪酸，且有辅助降糖作用，对糖尿病合并冠心病患者有益。

Clinical investigation of L-carnitine treating type-2 diabetic patients complicated coronary heart disease

Objective:To study the effect of L-carnitine on type-2 diabetic patients complicated coronary heart disease(CHD)and the influence of L-carnitine on blood glucose,lipid and free fatty acid(FFA)metabolism.Methods:The 62 patients with type-2 diabetic complicated CHD were randomly divided into two groups:Conventional treatment group and study group(treated with conventional treatment plus L-carnitine).Blood glucose,lipid and FFA levels were measured and those indexes between two groups,before and after treatment were analyzed.Results:The symptoms of myocardial ischemia,levels of lipid and FFA significantly decreased after treatment in study group and more than those of control group(P<0.05).Conclusion:It is useful to supply L-carnitine for the patients with type-2 diabetic complicated coronary heart disease;it can decrease the symptoms of ischemic myocardium and levels of lipid and FFA.
