切开挂线对口浮线引流术治疗高位肛周脓肿的临床观察

为观察切开挂线对口浮线引流术治疗高位肛周脓肿的临床疗效，将80例高位肛周脓肿患者随机分为治疗组（40例）和对照组（40例）。治疗组采用内口和脓肿低位切开、高位挂线、脓腔对口浮线引流术治疗，对照组采用切开引流术治疗。以一次性治愈率、术后肛周脓肿复发或形成肛瘘的二次手术率、住院时间、住院费用作为主要观察指标。结果显示，治疗组患者一次性治愈率明显高于对照组（92．5％vs20．0％，P〈0．05），术后肛周脓肿复发或形成肛瘘需二次手术率（7．5％vs80．0％，P〈0．05）、住院时间（28．0±5．5）dvs（43．0±6．3）d，P〈0．05]、住院费用明显低于对照组（P〈0．05）。随防2年，未发现肛门失禁、肛门移位、肛管直肠狭窄、肛门畸形及肛门功能障碍等并发症和后遗症。

Clinical Observation on the Combined Procedure of Incision,Thread-drawing and Orifice-to-Orifice Floating Thread-drawing Drainage in the Treatment of High Perianal Abscess

The objective of the study was to observe the therapeutic efficacy of the combined procedure of incision, thread-drawing and orifice-to-orifice floating thread-drawing drainage（ITDOOFTDD） in the treatment of high perianal abscess（HPA）. Eighty patients with HPA were randomized into two groups, patients in the study group （ n =40） underwent ITDOOFTDD,while patients in the control group（ n =40） underwent incision and drainage. Mainly observed such indexes as primary cure rate, postoperative abscess recurred or formed fistula which need second operation rate, hospital stay, hospitalization cost. As results, primary cure rate was markedly higher in the study group than in the control group（92.5% vs 20.0%, P 〈0.05） ,secondary operation rate （due to recurrence or fistula formation） in the study group was much lower（7.5% vs 80.0%, P 〈0.05） ,hospital stay in the study group was shorter（28.0±5.5） days vs （43. 0±6.3） days, P 〈0.05] ,and the hospitalization cost was lower in the study group （ P 〈0.05）. No such complications or sequelae as anal incontinence, anal displacement, anorectal stricture, malformation, of disfunction were found during 2 years of follow up.
