婴儿动脉导管未闭的介入治疗

目的 总结婴儿行动脉导管未闭(patent arterial duct,PDA)堵闭术的临床经验.方法 PDA 29例,年龄在1岁以下,体重4.5～10.5 kg,PDA最窄处直径2.0～7.4 mm,其中合并肺动脉高压13例,行蘑菇伞型封堵器封堵术.结果 所有病例1次封堵成功,术后即时升主动脉造影,发现有微量残余分流3例,术后3 d消失,1例有中量残余分流伴肉眼血尿,24 h后血尿消失,1个月后仅存微量残余分流,无其他并发症出现.结论 介入治疗对婴儿PDA疗效良好.

Successful closure of PDA in infants less than onc year old

Objectives To explore the feasibility, efficacy and some particularity of closing patent arterial duct in infants, using the Amplatzer duct occluder. Methods Twenty-nine cases aged less than one year old and weighing 4.5-10.5 kg were involved in the study. In 13 cases with middle-sever pulmonary hypertension, the diameter of narrowest PDA was 1.5-7.4 mm, 2 cases of them were intubated. The Amplatzer duct occluder was from 4-6 mm to 16-18 mm in size. Results All ducts were closed successfully in primary operation. Following descending aorta angiography, three cases were found to have small residual shunt which disappeared at the third days after operation. One case had middling residual shunt and hematuria which disappeared at 24 hour after operation. This case had small residual shunt at one month after operation, but it had no any complication. Conclusions Complete closure of patent arterial ducts with the Amplatzer ductus occluder in infants is the first choice method which is safe and effective.
