炔雌醇环丙孕酮联合螺内酯对多囊卵巢综合征患者激素的影响

摘    要:目的观察炔雌醇环丙孕酮(达英-35)联合螺内酯治疗多囊卵巢综合征(PCOS)对患者激素及体液潴留的影响。方法 54例PCOS随机分为观察组及对照组各27例,对照组给予"氯米芬+促卵泡成熟素"促排卵+达英-35治疗,观察组在此基础上加用螺内酯;比较两组排卵妊娠情况、临床表现、激素代谢水平以及水液代谢情况。结果①观察组出现周期排卵、单卵泡发育及妊娠的例数显著多于对照组(p<0.05);②激素水平方面,治疗后观察组促黄体生成激素(LH)、LH/FSH、催乳素(PRL)及睾酮(T)显著低于对照组(p<0.05);③临床表现方面,治疗后观察组的多毛、痤疮评分以及体重指数(BMI)显著低于对照组;④治疗后观察组的尿量显著增多并明显多于对照组(p<0.05),且血钠显著低于对照组,但血钾均在正常范围内且无组间差异。结论加用螺内酯可增强达英-35改善PCOS患者激素代谢紊乱症状,从而促进排卵及妊娠,且螺内酯还有改善体液的作用。
