艾滋病防治工作三级管理模式及效果评价

目的评价艾滋病防治市-镇-村三级管理模式的效果。方法依托市-镇-村三级疾控体系,建立以社区为基础的艾滋病防治工作三级医学管理模式,明确职责,规范随访管理、宣传干预等工作。结果模式建立前后,从事艾滋病防制的医务人员和管理病例的比例由1/296提高至1/10;艾滋病病毒感染者和艾滋病病人的流调率、阳性告知率、随访干预率分别从85.2%、85.2%和7.2%上升到94.0%、100%和64.9%;静脉注射吸毒者最近一次吸毒与别人共用针具率从4.6%下降至2.3%,最近一次非婚性行为时安全套使用率从21.7%上升至45.2%。结论建立市-镇-村三级的艾滋病防治工作医学管理模式,大大提高开展艾滋病防治工作的可及性,使艾滋病病毒感染者和艾滋病病人得到有效的管理,有效地减少艾滋病二代传播。

Evaluation of effectiveness of three levels AIDS management model

Aim To evaluate the effectiveness of three levels（City-town-village） AIDS control and prevention medical management model.Methods A communities-based three levels AIDS control and prevention medical management system was set up.Responsibilities were specified,instructions were given to guide follow-up management and interventions.Results Medical staffs engaged in AIDS control and prevention increased from 1/296 before the system was established to 1/10 thereafter.The rates for epidemiological survey ofHIV positives,the informed rate of HIV positives and follow-up intervention rate increased from 85.2%、85.2% and 7.2%85.2% to 94.0%、100% and 64.9%,respectively.The rate of sharing needles with others for the last use of drug dropped from 4.6% to 2.3% and the percentage of condom use for the last non-marital sex intercourse increased from 21.7% to 45.2% in intravenous drug users.Conclusion The accessibility of AIDS control and prevention has been greatly enhanced after setting up city-township-village AIDS management model and the second transmission of AIDS has been effectively controlled.
