低场强MR动态增强扫描对肝脏肿瘤的诊断与鉴别诊断

摘    要:目的:运用Gd-DTPA增强研究肝肿瘤低场强MR动态扫描及其价值。材料与方法:32例肝肿瘤39个病灶SE序列(TR/TE200~250/30ms)于1′、2′、5′、10′、20′分别作增强扫描,Gd-DTPA剂量0.1mmol/kg。结果:病例经手术病理和/或临床综合检查随访证实,包括原发性肝癌(HCC)22个病灶,血管瘤8个、囊肿5个,转移癌3个,局灶结节增生(FNH)1个。增强表现分为均匀增强(HCC 16/22、血管瘤2/8 FNH 1/1),延迟增强(血管瘤6/8,FNH1/1),不均匀增强(HCC 6/22、转移癌3/3)不增强(囊肿5/5)。结论:低场强MR短TR/TE SE序列可获得满意诊断图像;Gd-DTPA动态增强扫描对肝脏肿瘤诊断与鉴别诊断有重要价值。但目前此技术尚不能完全替代CT动态扫描。

Abstract:Purpose: To study the diagnostic value of contrast dynamic MRI of hepatic neoplasms. Materials and-Methods: 32 patients with hepatic tumor (39 lesions) induing 22 hepatocellular carcinomas (HCC), 8 hemangiomas, 5 cysts, 3 metastatic tumors', 1 focal nodular hyperplasias (FNH), confirmed pathologically and clinically, were scanned with contrast - enhancement dynamic SE (TR/TE,225/30ms) at 1', 2', 5', 10' and 20' after intravenous Gd-DTPA injection (0. ImmolAg). Results: All patients, The MRI features included homogeneous ehancement (HCC 16/22, hemangiomas 2/8, FNH 1/1), heterogeneous enhancement (HCC 6/22, metastatic tuomrs 3/3), delayed enhancement (heangiomas 2/8, FNH 1/1) and nonehancement (cyst 5/5) Conclution: The MRI imagings were good enough for the diagnostic purpose in such a low - field with short SE sequence: The Gd - DTPA dynamic enhanced MRI had a great value for the diagnosis and differential diagnosis of hepatic tumor. However it couldn't replace dynamic CT yet.
