耐甲氧西林金黄色葡萄球菌耐药性、mecA及6laTEM基因检测

摘    要:目的 了解临床分离的耐甲氧西林金黄色葡萄球菌(methicinin-resistant Staphylococcu8all—reus,MRSA)耐药性、mecA及6laTEM基因存在状况。方法采用ATBStaph药敏试验板微量肉汤法对40株MRSA进行15种抗菌药物敏感性测定,采用聚合酶链反应(PCR)技术检测mecA和6laTEM基因。结果 所有菌株对万古霉素、替考拉宁、呋西地酸和奎奴普丁-达福普汀均敏感。对青霉素、庆大霉素、红霉素、克林霉素、四环素、诺氟沙星和左旋沙星均耐药,对利福平、呋喃妥因、复方新诺明和米诺四环素的耐药率分别为5.0%、2.5%、97.5%和97.5%。mecA和blaTEM基因阳性率分别100.0%和37.5%。结论 临床分离的MRSA多重耐药性十分严重;金黄色葡萄球菌对甲氧西林耐药均由mecA基因介导.blaTEM基因阳性率较高;治疗MRSA引起的感染。应以药敏试验结果为依据。

Abstract:Objective To investigate the resistance, genes of mecA and blaTEM in methicillin-resitstant Staphylococcus aureus(MRSA). Methods Microdilute tests were performed to detect the susceptibility of 15 kinds of antimicrobial agents against 40 strains of the MRSA. Genes of mecA and blaTEM of these strains were analyzed by PCR. Results There were no strains resistant to Vancomycin, Teicoplanin, Fusidic acid, and Quinupristin-Dalfopristin, and no strains sensitive to Penicillin, Gentamicin, Erythromycin, Clindamycin, Tetracycline, Norfloxacin, and Levofloxacin. The resistant rates of 40 strains to Rifampicin, Furantoin, Trimethoprim-Sulfamethoxazole, and Minocycline were 5.0%, 2.5%, 97.5%, and 97.5%, respectively. The positive rates of genes of mecA and blaTEM were 100.0% and 37.5% in 40 strains. Conclusions The multiple-resistance of the MRSA is a serious issue. The resistance to methicillin in staphylococcus aureus is mediated by mecA gene, and is higher positive rate of gene blaTEM. To deal with the infection caused by the MRSA should be based upon the sensitive tests of antibiotics.
