Adult-to-adult Living Donor Liver Transplantation:Surgical techniques and innovations
Abstract:Abstract In response to critical organ shortage, transplant professionals have utilized living donors in an attempt to decrease the mortality rate associated with waiting in the liver transplant list. Living donor liver transplantation (LDLT) has been somewhat limited by the steep learning curve associated with developing a program. Although LDLT, is now widely applied to adult recipients and with many technical improvements, many controversies on donor safe, recipient safe, small-for-size syndrome, dual grafts liver transplantation, and Budd-Chiari syndrome exist. In this paper, authors reviewed and introduced their own experience on donor evaluation and selection, hepatic vein outflow reconstruction with collaterals formation, new strategy for biliary reconstruction, trans-splenic artery embolization for treating small-for-size syndrome, the new approach on dual grafts, and new operation for Budd-Chiari syndrome, ect. With surgical technique improvements and innovations, the outcome of LDLT recently achieves the same goal as deceased donor liver transplantation.
