晚发型胎儿生长受限胎盘组织碱性成纤维细胞生长因子表达及超微结构的观察

摘    要:目的:观察晚发型胎儿生长受限(fetal growth restriction,FGR)患者胎盘组织中碱性成纤维细胞生长因子(basic fibroblast growthfactor,bFGF)的表达及其超微结构的变化。方法:FGR组和足月正常妊娠组的胎盘组织标本各15例,应用免疫组织化学法检测bFGF的表达,2组标本各选取3例应用透射电镜观察其超微结构的变化。结果:2组均见bFGF表达于绒毛合体滋养层细胞、绒毛微血管内皮细胞和绒毛间质细胞的胞浆。FGR组胎盘滋养层bFGF的表达显著高于正常妊娠组(P<0.01)。FGR组胎盘合体结节增多,胎盘绒毛变性,基底膜增厚,微血管内皮可见突起及钙化砂粒体,自体胞质脱落。结论:FGR胎盘组织有超微结构病理改变,bFGF的表达可能是继发性增高。

Abstract:Objective: To observe the changes of basic fibroblast growth factor (bFGF) and ultrastructure in placental tissue of fetal growth restriction (FGR). Methods: Samples of placental tissues were collected from 15 FGR and 15 normal pregnant women. Immunohistochemistry was used to determine both location and intensity of bFGF staining in placenta. Three samples of FGR and 3 samples of control group were prepared for electron microscopic examination. Results: Positive expression of bFGF located in the cytoplasm of trophoblasts, villous stromal cells and capillary endothelial cells. The number of syncytial knots was increased in FGR group. The expression of bFGF was significantly higher in FGR group than that in the control (P〈0.01). Compared with the control, the placenta of FGR showed degenerative changes. The basement membrane was thickened. Microvilli-like projections, autoschizis and calcification psammomatous body were observed in the villous capillary endothelium. Conclusion: The placental tissue of FGR had ultrastructural pathological changes, and bFGF was secondarily increased.
