肝细胞癌中VEGF,HPA的表达与肿瘤微血管密度的关系及意义

摘    要:目的:探讨血管内皮生长因子(VEGF),乙酰肝素酶(HPA)在肝细胞癌(HCC)组织中的表达与肿瘤微血管密度(MVD)的关系及其意义。方法:应用免疫组织化学二步法联合检测72例HCC,62例肝硬化和23例正常肝组织中VEGF,HPA的表达和进行MVD计数,结合临床病理资料进行分析。结果: HCC中VEGF,HPA的表达率均明显高于肝硬化及正常肝,HCC中MVD值也显著高于肝硬化及正常肝。VEGF,HPA的表达率在TNMⅠⅡ期、无转移复发组、AFP<400μg/L组、无门脉癌栓组均明显低于ⅢⅣ期、转移复发组、AFP≥400μg/L组、有门脉癌栓组。在TNMⅠⅡ期、无转移复发组、AFP<400μg/L组、无门脉癌栓、单个肿瘤结节以及肿瘤直径<5cm组MVD计数亦明显低于ⅢⅣ期、转移复发组、AFP≥400μg/L组、有门脉癌栓、多个肿瘤结节以及肿瘤直径≥5cm组。VEGF,HPA的表达率与MVD值三者两两之间显示明显正相关。结论: VEGF,HPA在促进肿瘤微血管生成中起重要的作用,联合检测VEGF,HPA,CD34在肝癌中的表达,对肝癌患者的预后评估有一定的辅助意义。

Abstract:Objective:To investigate the relationship of the expression of vascular endothelial growth factor(VEGF),heparanase(HPA)protein and the microvessel density(MVD)in hepatocellular carcinoma(HCC)tissues and its significance.Methods:Immunohistochemistry was employed to detect the expression of VEGF and HPA and the numbers of MVD in 72 cases of HCC,62 cases of cirrhosis and 23 cases of normal liver tissues.Statistic was analyzed together with the clinicopathological parameters.Results:The positive rate of VEGF and HPA in HCC tissues was significantly higher than that in cirrhosis and normal liver tissues;The MVD in HCC tissues was significantly higher than that in cirrhosis and normal liver tissues.The positive rate of VEGF,HPA in HCC tissues in the clinical TNM stageⅠ,Ⅱand that in the cases without metastasis,AFP<400μg/L and without tumor embolus were significantly lower than those in the stage Ⅲ,Ⅳ,with metastasis,AFP≥400μg/L and portal vein tumor embolus;The MVD in HCC tissues in the clinical TNM stageⅠ,Ⅱ and that in the cases without metastasis,AFP<400μg/L,without tumor embolus,single tumor node and the diameter of tumor <5cm,were significantly lower than those in the stage Ⅲ,Ⅳ,with a metastasis,AFP≥400μg/L,portal vein tumor embolus,multiple nodes and the diameter ≥5cm.There was positive correlation between the expression of VEGF and HPA and the MVD.Conclusions:The over-expression of VEGF and HPA may play an important role in stimulating tumor neovaseularization.Combined detection of the VEGF and HPA expressions may be helpful in predicating the biological behavior of HCC patients.
