胰岛素样生长因子-Ⅰ与高血压左心室肥厚的相关性

摘    要:目的探讨原发性高血压患者血清胰岛素样生长因子Ⅰ(IGF-Ⅰ)水平与左心室肥厚(left ventricu-lar hypertrophy,LVH)的关系。方法采取酶联免疫分析方法测定51例原发性高血压患者和30名正常人的血清IGF-Ⅰ水平。根据心脏多普勒超声各项参数计算左心室重量指数(left ventricular mass index,LVMI)。用单因素方差分析比较正常对照组、单纯高血压(EH)组和高血压合并左心室肥厚(EH-LVH)组LVMI、IGF-Ⅰ差别,用Pearson直线相关分析血清IGF-Ⅰ与LVMI的相关性。结果EH组血清IGF-Ⅰ高于正常对照组(P<0.05),EH-LVH组血清IGF-Ⅰ高于EH组(P<0.05),并显著高于正常对照组(P<0.01);血清IGF-Ⅰ与LVMI存在正相关(r=0.45,P<0.05)。结论循环血IGF-Ⅰ水平与高血压及高血压合并左心室肥厚密切相关,表明其参与了高血压病的发生发展,且可能在高血压病心肌重构过程中起到了重要作用。

Abstract:Objective To investigate the relationship between insulin-like growth factor-Ⅰ (IGF-Ⅰ) and left ventricular hypertrophy (LVH) in patients with essential hypertension (EH). Methods In 30 normal individuals and 51 patients with EH, the serum level of IGF-Ⅰ was measured using enzyme-linked immunoassay, and left ventricular mass index (LVMI) was calculated based on parameters determined by echocardiography. Differences in LVMI and serum IGF-Ⅰ among groups of normal controls, patients with EH alone or LVH-complicated EH (EH-LVH) were analyzed with univariate ANOVA. Pearson′s linear correlation was employed to identify the possible relationship between serum IGF-Ⅰ and LVMI. Results The IGF-Ⅰ level was significantly higher in EH patients than in normal controls (P<0.05), as was in EH-LVH patients than in EH patients (P<0.05) or than in normal controls (P<0.01). The level of IGF-Ⅰ showed positive correlation with LVMI (r=0.45, P<0.05). Conclusion The positive correlation between serum IGF-Ⅰ and EH or EH-LVH suggests that IGF-Ⅰ may be involved in the pathophysiological process of essential hypertension, and probably play a very important role in the development of myocardial remodeling.
