支气管动脉灌注联合心包腔导管留置引流治疗肺癌心包积液

摘    要:目的为观察原发性支气管肺癌心包积淀,采用支气管动脉插管化疗药物灌注联合心包腔导管留置引流治疗效果。方法采用经皮穿刺选择性支气管动脉插管化疗药物灌注,每次经导管注入表阿霉素30mg、卡铂300mg、羟基喜树碱6mg以及白细胞介素-Ⅱ30万U,每3、4周介入治疗1次,心包积液采用经皮心包腔导管置入间歇引流,并经导管注入少量化疗药物。结果14例患者共进行支气管动脉插管药物灌注48次,最多6次,最少1次,平均3.4次,心包腔穿刺导管置入最长35天,最短7天,平均15天。每次导管引流心包液最多800ml,最少40ml,平均250ml。本组CRS例,PR4例,有效率为64.3%。4例存活1年以上,7例存活8个月以上,2例存活4个月,1例存活不到3个月。结论肺癌心包积液采用支气管动脉插管药物灌注与心包腔导管留置间歇引流是有效的治疗方法

Abstract:Objective: To observe the therapeutic effect in the treatment of hydropericardium of primary bronchogenic carcinoma,using bronchial artery cannula infusion with chemotherapy drugs and drainage from indwelling catheter in pericardium. Methods The patients were given percutaneous bronchial artery cannula infusion every 3-4 weeks, Hydropericardium was drained by indwelling catheter in pericardium. Coupled with small dose of chemotherapy drugs from this catheter. Results 14 cases were given infusion 48 times altogether with the maximum of 6 times and minimum once. The staying time of the indweiling catheter in pericatdium ranged from 35 to 7 days,and the quanlity of draining fliud each time was 800ml to 400ml. The group under study included 5 cases of CR and 4 cases of PR. The effective rate was 64. 3%. with 4 cases surviving for more than 1 year,7 for over 8 months,2 for 4 months and 2 for less than 3 months. Conclusion It is an effective way for the lung cancer with pericardium metastasis to be treated through bronchial arterial intervention therapy and intermittent.
