我院心外科249例围术期患者抗菌药物应用分析

目的:评价我院心外科围术期患者抗菌药物的应用情况。方法:采用回顾性调查方法,对我院2009年1～6月心外科术后249例患者抗菌药物的应用情况进行统计、分析。结果:249例患者中,术前预防性应用抗菌药物244例,术后应用抗菌药物249例。术前共应用5类7种抗菌药物,应用率最高者为青霉素钠(146例),其次为头孢噻肟钠(101例)。二联用药共22例,无三联及以上用药现象。病原学检查与药敏试验仅30例。初次预防给药时间<30min;预防用药时间最短1d,最长15d,平均5.87d;手术持续时间>3h者71例,<3h者178例,无术中追加抗菌药物情况。结论:我院心外科围术期患者应用抗菌药物存在品种选择不当、给药时机不合理、用药时间过长、病原学检查不足等情况,亟待干预。

Analysis on 249 Cases of Perioperative Application of Antibiotics in Cardiac Surgery Department of Our Hospital

OBJECTIVE：To evaluate the perioperative application of antibiotics in cardiac surgery department of our hospital.METHODS：By retrospective analysis,249 cases of perioperative application of antibiotics in cardiac surgery department of our hospital from Jan.2009 to Jun.2009 were analyzed statistically.RESULTS：Among 249 patients,244 patients received antibiotics before operation and 249 after operation.5 categories（7 kinds）of antibiotics were adopted before operations.Benzylpenicillin sodium occupied the highest proportion（146 cases）,followed by cefotaxime sodium（101 cases）.22 cases received two-drug therapy.;none of case used 3 kinds or more antibiotics.Only 30 cases have been received etiologic test and susceptibility test.The initial delivery time before operation was lower than 30 min.The shortest delivery time of antibiotics before operation was 1 d and the longest was 15 d with an average time of 5.87 d.The operations duration of 71 cases were more than 3 h and 178 cases lower than 3 h.None of case received additional antibiotics during the operations.CONCLUSION：There are some problems to deal with,such as inappropriate selection of drug category,unreasonable timing,delayed medication time and poor etiology test,etc.
