东菱迪芙与其它药物治疗急性脑梗死对照研究的Meta分析

目的:了解东菱迪芙(巴曲酶)与其它药物治疗急性脑梗死的临床疗效差异。方法:应用固定效应模型的Meta分析对8项研究结果进行同质性检验和合并效应量的估计,用漏斗图分析发表性差异。结果:同质性检验:χ2=9.32,自由度为7,P>0.05;合并效应量的估计:OR合并=3.67,OR合并95%可信区间为2.46～5.47;OR合并的检验:χ2=40.69,P<0.01。结论:东菱迪芙治疗急性脑梗死有较确切的疗效,但尚应进行严格的、多中心的随机双盲对照试验,以提供更具说服力的证据。临床科研设计中仍存在较多问题亟待改善,如随机和盲法的严格正确实施、判效指标的选择、随访及临床试验报告规范化等,以及其在适应证、治疗剂量、给药方法及用药时限等方面与临床疗效的关系尚有待进一步研究。

Meta-analysis of Batroxobin and Other Drugs in Treatment of Acute Cerebral Infarction

OBJECTIVE：To compare clinical efficacy of batroxobin and other drugs in treatment of acute cerebral infarction.METHODS：Meta-analysis of fixed effects model was used to evaluate homogeneity test and combined OR.Funnel plot uas used to analyze the difference of publication.RESULTS：Results of homogeneity test showed χ^2=9.32,free degree 7,P〉0.05;combined OR=3.67;95% confidence interval of combined OR was ranged from 2.46 to 5.47（χ^2=40.69,P〈0.01）.CONCLUSION：Batroxobin has sound effect on acute cerebral infarction.Multi-center double-blinded randomized controlled trials（RCT） are required to provide more strong evidence.Many questions existing in clinical trials need to be resolved,such as the implementation of double-blinded RCT,selection of criteria,standard follow-up report and experiment report,etc.And it is necessary to further investigate the relationships between clinical efficacy of batroxobin and indication,dosage,medication method,medication duration.
