甘露聚糖肽干预拉米夫定耐药的慢性乙型肝炎疗效观察

目的观察甘露聚糖肽联合阿德福韦酯治疗拉米夫定耐药的慢性乙型肝炎疗效。方法采用非随机同期对照试验,将45例拉米夫定治疗产生耐药的患者分为治疗组和对照组,治疗组24例,对照组21例。治疗组给予甘露聚糖肽10mg一日三次,同时口服阿德福韦酯10mg,每天一次,治疗12个月。对照组给予口服阿德福韦酯10mg,每天一次,治疗12个月。观察患者临床症状、ALT复常率、HBeAg转阴率、HBeAg血清转换率、HBVDNA转阴率。结果治疗3、6、12个月时,ALT复常率治疗组分别为67.8%、84.3%、90.5%,对照组分别为51.2%、63.2%、79.6%,两组比较x2值分别为9.6、12.4、8.7,P值均〈0.01,差异有统计学意义。治疗组6、12个月HBVDNA阴转率、HBeAg阴转率、HBeAg血清转换率均优于对照组,两组比较,P值均〈0.01,差异有统计学意义。结论甘露聚糖肽联合阿德福韦酯治疗拉米夫定耐药的慢性乙肝能有效地改善肝功能,提高阿德福韦酯抑制HBV复制。
