钛板坚强内固定术用于颌面骨折治疗的临床研究

目的研究钛板坚强内固定术用于颌面骨折治疗的临床效果。方法选择某院2008年7月～2011年1月采用手术治愈的125例颌面骨折患者做为研究对象,按随机分组原则分为钛板固定组(简称钛板组),和钢丝固定组(简称钢丝组),所有患者术前行CT及X片检查并进行其他常规功能检验,术后每月检查CT和X片、咬合检查。其中65例钛板组患者采取钛板坚强内固定手术,60例钢丝组患者采取传统手术加不锈钢丝固间固定手术。对比两组患者术后1～6个月内X线检查骨折的对位情况,骨折延期者继续跟踪复查直至愈合完好。结果钛板组咬合成功率(63例)96.9%,钢丝组成功咬合率(52例)86.7%,钛板组手术成功率明显高于钢丝组,组间差异具有统计学意义(P﹤0.01)。钛板组手术失败2例者中1例由于钛板断裂于术后6个月取出钛板延期愈合、1例骨折感染经局部换药对症治疗伤口2期愈合。结论钛板坚强内固定术可做为治疗颌面骨折的有效治疗方式,它可以使骨折部位咬合,达到坚固的理想效果;但针对于钛板的存留问题,还有待于临床大量研究

Observation on the clinical effect of titanlum miniplates used in maxillofacial surgery

OBJECTIVE To observe the clinical effect of titanlum miniplates in treating the patients with maxillofacial fracture.METHODS Selected 125 cases with maxillofacial fracture from July 2008 to January 2011 which were divided into 2 groups with study group 65 cases,control group 60 cases.Before surgery,all patients took CT and X-ray as well as some routine tests on body function,after surgery,CT and x-rays monthly were given,as well as the inspection of occlusion.The study group would have an titanlum miniplates in maxillofacial surgery,while control group would take posterior wiring and bone grafting.RESULTS There were significantly differences between the two groups in successful rates which study group showed an obvious effect that(63 cases)96.9% cases had perfect occlusion,whereas control group took(52 cases)86.7%.2 cases of study group were in failure for titanium plate fracture and fracture infection.CONCLUSION Titanlum miniplates used in maxillofacial surgery can be of great use and clinical effect,however the titanium plate whether to remove after surgery remains to studied in clinic.
