腹腔镜结肠癌根治性切除术58例临床观察

目的 探讨腹腔镜结肠癌根治性切除术的临床效果观察.方法 选择某院自2010年2月～2012年1月的58例结肠癌患者作为治疗组,按照患者的自愿原则,将患者分为治疗组与对照组,每组29例.治疗组采用腹腔镜手术,对照组采用常规的开腹手术.对两组的术后生存率、手术时间、术中出血量、术后并发症以及肠道功能恢复情况进行对比分析.结果 ①治疗组与对照组的术后生存率分别为93.10％ (27/29)、89.66％ (26/29),两组比较差异无统计学意义(P＞0.05)；②两组手术时间、术中出血量分别为(207.4±22.4) min、 (174.5±14.8) min, (106.5±25.7) ml、(203.2±34.1) ml,两组比较差异有统计学意义(P＜0.01)；治疗组术后并发症的发生率低于对照组,患者肠道功能恢复情况显著高于对照组(P＜0.05).结论 腹腔镜结肠癌根治切除术具有安全、术后恢复快、并发症较少等优点,具有与常规开腹手术相当的远期生存率,可以安全的用于结肠癌根治的治疗,是一种值得临床上推广采用的技术.

Clinical observation of laparoscopic colon cancer resection in 58 cases

OBJECTIVE To investigate the clinical effect of laparoscopic colon cancer resection.METHODS 58 colon cancer patients with laparoscopic colon cancer resection from February 2010 to January 2012 were chosen.They were divided into the treatment group and the control group(n = 29 cases).The treatment group had laparoscopic surgery,and the control group had conventional open surgery.The survival rates,operative time,blood loss,postoperative complications,and bowel function recovery of the two groups were compared and analyzed.RESULTS ① The survival rate of the treatment group and the control group were 93.10%(27/29),89.66%(26/29).There was no statistical difference(P﹥0.05);② Surgical time and intraoperative bleeding of the 2 group respectively were(207.4 ± 22.4)min and(174.5 ± 14.8)min,(106.5 ± 25.7)ml(203.2 ± 34.1)ml,the difference was significant(P﹤0.01).The incidence of postoperative complications in the treatment group was lower than the control group,and the recovery of bowel function in patients was significantly higher(P﹤0.05).CONCLUSION The laparoscopic colon cancer resection has the advantage of safety,faster recovery,fewer complications and long-term survival rate.It can be safely used for the treatment of colon cancer.
