彩色多普勒超声对原发性肝癌患者放疗后的疗效评估

目的探讨彩色多普勒超声在原发性肝癌患者放疗后的的应用价值。方法随访2008年10月～2010年6月某院收治的36例行三维适形放疗的原发性肝癌患者,在治疗前后均实施彩色多普勒超声检查方法 ,观察血流动力学变化。结果放疗前后彩色多普勒超声血流动力学检查项目包括周边收缩期峰值流速(cm/s)、周边舒张期最低流速(cm/s)、周边阻力指数、周边门脉样流速(cm/s)、内部收缩期峰值流速(cm/s)、内部舒张期最低流速(cm/s)、内部阻力指数、内部门脉样流速(cm/s),其中周边收缩期峰值流速(cm/s)、周边舒张期最低流速(cm/s)、内部收缩期峰值流速(cm/s)、内部舒张期最低流速(cm/s)放疗后显著低于放疗前(P﹤0.05),而周边阻力指数、周边门脉样流速(cm/s)、内部阻力指数、内部门脉样流速(cm/s)放疗前后差异无统计学意义(P﹥0.05)。结论彩色多普勒超声具有操作简单、无创的优点,应用于行三维适形放疗的原发性肝癌患者的疗效评估,诊断价值高,值得临床上推广应用。

Evaluation on curative effect of Color Doppler ultrasonography in primary hepatic carcinoma patients after radiotherapy

OBJECTIVE To explore the application value of the color Doppler ultrasound in patients with primary liver cancer after radiotherapy.METHODS From October 2008-June 2010,36 primary liver cancer patients given three-dimensional conformal radiotherapy were admitted to our hospital,the implementation of the color Doppler ultrasonic examination method was carried out before and after the treatment,then observed the hemodynamic changes.RESULTS Radiotherapy and color Doppler ultrasonography hemodynamic examination items included surrounding the peak systolic velocity(cm/s),peripheral minimum diastolic velocity(cm/s),peripheral resistance index,peripheral and portal flow rate of sample(cm/s),internal peak systolic velocity(cm/s),minimum diastolic velocity(CM/s),internal resistance index,sector pulse sample flow velocity(cm/s),in which the peripheral peak systolic velocity(cm/s),peripheral minimum diastolic velocity(cm/s),internal peak systolic velocity(cm/s),minimum diastolic velocity(cm/s)were significantly lower than before radiotherapy(after radiotherapy P﹤0.05),and peripheral resistance index,peripheral and portal flow rate of sample(cm/s),internal resistance index,sector pulse sample flow velocity(cm/s)before and after radiotherapy had no significant difference(P﹥0.05).CONCLUSION Color Doppler ultrasound is simple and noninvasive,application in evaluation of curative effect of three-dimensional conformal radiotherapy for primary hepatocellular carcinoma patients has the high diagnostic value,which is worthy for clinical application.
