经阴道超声联合肿瘤标志物检测对卵巢良恶性肿瘤的诊断分析

摘    要:目的研究经阴道超声联合肿瘤标志物检测对卵巢良恶性肿瘤的诊断价值。方法选择某院2008年5月~2011年5月收治的120例卵巢肿瘤患者作为研究对象,其中恶性肿瘤10例,良性肿瘤110例,所有患者均采用阴道超声检查后记录其肿块的超声征象,同时采用化学发光法检查患者体内肿瘤标记物CA125的含量,并对结果行统计学分析。结果阴道超声检出的准确率为90%(108),经CA125标记物检出的准确率为84.16%(101),两者联合诊断的准确率为95%(115),联合诊断准确率显著高于二者单独使用,差异有统计学意义(χ2=1.595,P﹤0.01)。结论经阴道超声检查卵巢良恶性肿瘤,二维图像可清晰观察肿瘤内部结构及彩色多普勒观察肿瘤血流分布及频谱特征,结合检测外周血中CA125水平可以更为准确地评价卵巢肿瘤的性质,从而弥补单独使用两种方法之一的不足。阴道超声联合肿瘤标志物对卵巢肿瘤可以较好地鉴别出良恶性肿瘤的性质,值得广泛用于临床使用。

Abstract:OBJECTIVE To study the transvaginal ultrasound combined detection of tumor markers in the diagnosis of benign and malignant ovarian tumors. METHODS 120 cases of patients with ovarian tumors were selected in our hospital from 2008 May to 2011 May. 10 cases had malignant tumor, and 110 patients had benign tumor. All patients were treated with vaginal ultrasound examination after recording the mass ultrasound signs. Chemiluminescence examination was used for patients with vivo tumor marker CA125 content. Statistical analysis was used for them. RESULTS The detection accuracy rate of vaginal ultrasound was 90% (108), accuracy rate of CA125 markers detection was 84.16% (101), and both of the diagnostic accuracy rate was 95% (115). The diagnostic accuracy was significantly higher than the two single detection method (χ2 = 1.595, P﹤ 0.01). CONCLUSION The benign and malignant ovarian tumors by transvaginal ultrasonography, two -dimensional image can be clearly observed in tumor internal structure, and color Doppler could observe tumor blood flow distribution and spectrum characteristics. Combined with peripheral blood CA125 levels can be more accurate for detecting the ovarian tumor nature, which could make up the defects of the two methods. Vaginal ultrasound combined with tumor marker for epithelial ovarian tumors can better identify benign and malignant tumor nature, which is worth of widely used in clinical use.
