Roux-en-Y与Billroth重建术式在胃切除术中的应用

目的探讨Roux-en-Y与Billroth重建术式在胃切除术中的临床应用价值。方法选择某院2006年10月～2010年3月收治的拟行远端胃切除患者117例,分别采用Roux-en-Y术,Billroth-I术,Billroth-Ⅱ术进行远端胃切除术,比较3组手术效果及并发症情况。结果 Billroth-Ⅱ组的手术时间、拔管时间、口服饮食时间、住院天数均较Billroth-I组高(P﹤0.05);Roux-en-Y组的住院时间较Billroth-I组长(P﹤0.05);Billroth-Ⅱ组各种并发症的发生情况较Roux-en-Y组或Billroth-I组高(P均﹤0.05);3组之间各随访时间点生存率差异无统计学意义(P﹥0.05)。结论 Roux-en-Y术式优于Billroth-Ⅱ术式,但未发现Roux-en-Y术式优于Billroth-I术式。

Application of Billroth and Roux-en-Y reconstruction in gastrectomy

OBJECTIVE To investigate the clinical application value of Roux-en-Y and Billroth reconstruction on gastric resection. METHODS From October 2006 to March 2010, 110 patients admitted in our hospital intended to implement total gastrectomy, Roux-en-Y, Billroth-I and Billroth-Ⅱ of digestive tract reconstruction were applied respectively. Surgical outcomes and complications were compared. RESULTS For the Billroth-Ⅱ group, operating time, extubation time, oral diet time and inhospital time were higher than Billroth-I group (P﹤0.05); Compared with Billroth-I, hospital time for Roux-en-Y group was longer (P﹤0.05); The complications for Billroth-Ⅱ were signifciantly higher than Roux-en-Y or Billroth-I groups (P﹤ 0.05); No statistical significance were found in survival rates between the three groups (P﹥0.05). CONCLUSION Roux-en- Y was better than Billroth-Ⅱ, but not Billroth-I.
