脑出血患者细胞免疫功能变化研究

目的探讨红细胞免疫促进因子、免疫抑制因子及T淋巴细胞亚群免疫黏附功能的变化及规律,为今后临床病情判定、预后的预测提供依据。方法选择脑出血患者及正常对照组进行红细胞C3b受体花环率(RBC-C3bRR)、红细胞免疫复合物花环率(RBC-ICR)、红细胞免疫促进因子(RFER)、红细胞免疫抑制因子(RFIR)以及T淋巴细胞亚群(CD3+、CD4+、CD8+)的测定,并比较两组结果的差异。此外,将患病组进行神经功能的损伤程度分层比较。结果病例组与对照组RBC-C3bRR、RBC-ICR、RFER及RFIR的差异有统计学意义(P﹤0.001);按照神经功能损伤程度分层后比较,不同损伤程度RBC-C3bRR、RBC-ICR、RFER及RFIR的差异也有统计学意义(P﹤0.01)。病例组与对照组CD3+、CD4+、CD8+及CD4/CD8的差异有统计学意义(P﹤0.01);按照神经功能损伤程度分层后比较,不同损伤程度CD3+、CD4+、CD8+及CD4/CD8的差异也有统计学意义(P﹤0.01)。结论脑出血患者的红细胞免疫促进因子、免疫抑制因子及T淋巴细胞亚群免疫功能较为低下,检测这些细胞学指标对判断病情,预后的预测有一定的指导意义。

Study on the cellular immune function among patients with cerebral hemorrhage

OBJECTIVE To explore the changes and trend of RCIA of RFER,TDSF,CD3+,CD4+ and CD8+,so as to provide clinical reference for disease determination and prognosis prediction.METHODS We determined the RBC-C3bRR,RBC-ICR,RFER,RFIR,CD3+,CD4+ and CD8+,and compared the difference of the two groups.Besides,we performed the stratified analysis according to injury status of nerve function.RESULTS The difference of RBC-C3bRR,RBC-ICR,RFER and RFIR showed significant difference between cases and controls.According to injury status of nerve function,there was significant difference in RBC-C3bRR,RBC-ICR,RFERand RFIR(P﹤0.01).There was significant difference in the CD3+,CD4+,CD8+ and CD4/CD8 between the two groups(P﹤0.01).According to the injury status of nerve function,there was significant difference in CD3+,CD4+,CD8+ and CD4/CD8 between the two groups(P﹤0.01).CONCLUSION The RCIA of RFER,TDSF,CD3+,CD4+ and CD8+ showed significant lower function in patients with cerebral hemorrhage.These indexes has a certain value in diagnosis of the disease,and could provide a reference for diagnosis.
