边远民族地区村医队伍现状调查

目的了解边远民族地区村医人员数量、结构、分布、培训、收入与社会保障现状。方法在贵州省黔东南苗族侗族自治州凯里市10乡镇2街道办事处165个村卫生室对所有在岗村医进行问卷调查。结果村医共183人,平均每村村医0.91人,每村乡村医生和卫生员0.89人,每千农业人口乡村医生和卫生员0.62人;近5年28名村医流失,占现有村医总数的15.3%。村医年龄为19～71岁,平均39.5岁;男性村医占70.5%,女性村医占29.5%;乡村医生占97.3%,执业(助理)医师占2.7%;无专业技术职称者占97.8%,初级职称者占2.2%;中专医学学历者占62.8%,大专学历者占2.2%,无医学学历者占35.0%;村医每人月均收入1 350元;无村医养老保险;各乡镇村医农业人口分布的洛伦兹曲线紧靠绝对公平线。结论凯里市各乡镇村医分布公平性较好;然而,村医短缺;年龄结构偏老化;专业技术水平待提高;基本待遇待改善。

Investigation on the situation of village health personnel in remote minority areas

OBJECTIVE To investigate the current situation of human resources for village health personnel(VHP)in terms of quantity,structure,distribution,training,income and social welfare in remote minority areas.METHODS Personal interview for VHP was conducted with structured questionnaire in 165 village’s clinics of Kai Li in Qiandongnan autonomous prefecture of Miao and Dong minorities in July 2010.RESULTS A total of 183 VHPs was reported in Kai Li,with an average of 0.91 person of VHPs per village,0.89 village doctor and assistant per village,and 0.62 village doctor and assistant per 1000 rural population.About 28 VHPs resigned their jobs in the recent five years,accounting for 15.3% of the current total number of VHPs.The age of VHPs was ranged from 16 to 71 years,with an average age of 39.5 years.About 97.3% of respondents were village doctors as occupation qualification,and 2.7% were medical doctors or assistants.97.8% of respondents did not have any title of technical post,and 2.2% had a primary title of technical post.With regard to medical education,technical secondary school or relevant certificate accounted for 62.8%,junior college for 2.2%,and none for 35.0%.The respondents reported the average monthly medical service income was 1350 Yuan in 2009.All respondents did not have VHP endowment insurances.Lorenz curve of VHP distribution was close to absolute equity line.CONCLUSION VHP distribution showed a good equity in Kai Li.However,VHP was in a shortage with an older age structure.The technical capability should be improved,and a basic benefit should be provided for VHP.
