脑外伤所致精神障碍的相关影响因素分析

目的探讨精神障碍发生的相关影响因素,为今后临床工作中采取针对性的治疗及干预措施提供依据。方法选择某院门诊或住院治疗的脑外伤所致精神病患者78例,对照组为同期就诊的未出现精神障碍的脑外伤患者80名作为研究对象。采用非条件Logistic回归,分析脑外伤所致精神障碍的相关影响因素。结果单因素分析病例组与对照组调查对象发生颅内血肿、脑干损伤的比例以及脑损伤范围、GCS评分、EPQ评分的差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析,GCS评分及EPQ评分差异有统计学意义,GCS评分越高,发生精神障碍的可能性越低;而EPQ得分越高,发生精神障碍的可能性越低。颅内血肿、脑干损伤及脑损伤范围与脑外伤后精神障碍的发生无关联性。结论脑外伤后昏迷程度及人格倾向是发生精神障碍的影响因素,临床诊疗中应采取针对性的干预措施。

Analysis of the influencing factors on the mental disorders among patients with cerebral trauma

OBJECTIVE To explore the related influencing factors of the mental disorders among patients with cerebra trauma,so as to provide related treatment and intervention measures for clinical works.METHODS A total of 78 patients with cerebra trauma and mental disorders were collected,and 80 patients without mental disorders were collected as controls.By unconditional Logistic regression,we analyzed the related factors of mental disorders.RESULTS The univariate analysis showed there was a significant difference between the proportion of intracranial hematoma and brainsterm injury,brain damage range,GCS scores and EPQ scores(P﹤0.05).The multifactor regression analysis showed there was a significant difference in the GCS and EPQ scores,and the higher GCS score was related to the high possible of mental health disorder.While the higher EPQ score was related to the lower mental health.There was no significant difference in the range of intracranial hematoma and brainstem injury and occurrence of cerebra trauma.CONCLUSION The influencing factors for the mental health after cerebra trauma are degree of coma and personality trend.We should take related factors for clinic diagnosis and treatment.
