三维适形放疗在食管癌纵隔转移淋巴结治疗中的临床应用效果及其安全性评估

目的 探讨三维适形放疗在食管癌纵隔转移淋巴结治疗的临床价值.方法 收集2006年1月～2008年1月间某院治疗的资料完整的182例食管癌纵隔转移淋巴结患者临床资料,行三维适形放疗的120例为观察组,其余接受常规放疗治疗的62例为对照组,比较两组的近期疗效、毒副作用及近、远期生存率.结果 行三维适形放疗观察组患者CR比例为53.3％,PR比例为35.0％,缓解率为88.3％,均显著高于对照组；而3年生存率为21.7％,也显著高于对照组,差异均有统计学意义(P＜0.05).观察组患者治疗后,急性放射性食管炎、急性放射性气管炎及急性放射肺炎等并发症为Ⅲ和Ⅳ的比例分布为13.4％,17.5％,14.1％,均较对照组显著降低(P＜0.05).结论 三维适形放疗可显著提高患者生存率,且治疗后毒副反应降低.

Clinical effect and safety assessment of three-dimensional conformal radiotherapy in the treatment of lymph node metastases in esophageal cancer

OBJECTIVE To investigate the clinical value of three-dimensional conformal radiotherapy in mediastinal lymph node metastasis in esophageal cancer treatment.METHODS From January 2006-January 2008,182 cases of esophageal carcinoma with mediastinal lymph node metastasis were collected in our hospital.They were divided into 2 groups.The observation group with 120 cases had three-dimensional conformal radiotherapy,and the control group with 62 cases had conventional radiotherapy.Short-term efficacy,side effects and survival rate were compared.RESULTS Complete remission rate and partial remission rate were 53.3% and 35.0% in the observation group which were significantly higher than the other group;3-year survival rate was 21.7% in the observation group,which was also significantly higher(P﹤0.05).The rates of acute radiation esophagitis,acute radiation tracheitis and acute radiation pneumonia,were 13.4%,17.5%,and 14.1%,which were lower than the control group(P﹤0.05).CONCLUSION Three-dimensional conformal radiation therapy can significantly enlighten survival rate and reduce side effects.
