老年严重多发伤急诊救治结局分析

目的分析老年严重多发伤急诊救治结局的影响因素,探讨提高老年严重多发伤急诊救治成功率的措施。方法回顾性分析237例老年严重多发伤急诊患者的诊治资料,按照救治结局的不同将其分为4组,统计4组患者的一般资料、急诊时相关联动科室情况、不同年份患者救治结局,分析患者因素、医疗因素等对老年严重多发伤急诊救治结局的影响。结果经过分析发现,高龄、多发伤发生前合并疾病多、创伤严重程度高、创伤发生至首次急诊手术时间越长是导致患者救治结局差的主要原因;同时,院前急救指导、患者就诊时的相关联动科室工作效率也对患者救治结局有重要影响。随着时间的推移,医疗设备的添置,急诊救治人员技术的不断提高,不同年份的老年严重多发伤患者的急诊救治结局存在明显的差异,不同年份之间治愈出院率比较,差异有统计学意义(P﹤0.05)。结论影响老年严重多发伤急诊救治结局的主要因素是患者因素和医疗因素,通过改善急诊管理流程、加强急诊设备配备、人员培训等方式可有效改善老年严重多发伤的急救结局。

Analysis of the outcome of emergency rescue of elderly with severe multiple trauma

OBJECTIVE To analyze the risk factors of the outcome of emergency rescue of elderly with severe multiple trauma,and explore measures of improving the treatment success for elderly with severe multiple trauma.METHODS The data of 237 cases of elderly patients with severe multiple trauma emergency room diagnosis and treatment were analyze retrospectively.According to the different treatment outcome,they were divided into four groups.The general information,emergency situation for the relevant sections of the linkage and the outcome of patients in different years for the four groups were analyzed.The patient factors and medical factors of the outcome of elderly with severe multiple trauma outcome were analyzed.RESULTS The analysis found that age,multiple trauma disorders before the merge,high trauma severity and the first occurrences of emergency surgery in patients with longer treatment were the risk factors of the poor outcome.At the same time,pre-hospital aid guide and treatment related department for patients had a major impact on the efficiency of treatment outcome of patients.Over time,the purchase of medical equipment,continued increased of personnel technology for emergency medical treatment and different years of age showed different outcome of severe multiple trauma with emergency treatment showed significant difference between the discharged rates(P﹤0.05).CONCLUSION The major factors for the poor outcome of severe multiple trauma were from the patients and medical measures.By improving the emergency management process,we should improve the outcome of severe multiple trauma by improving emergency equipment,equipment and training of personnel.
