急性脑梗死合并上消化道出血的危险因素及预后分析

目的了解急性脑梗死并发上消化道出血的危险因素及预后。方法对徐州医学院第二附属医院收治的42例急性脑梗死并发上消化道出血患者的资料进行临床研究。结果意识障碍的患者发病率高,并发上消化道出血患者幽门螺杆菌（Hp）感染率较高,临床上多在病程7 d以内出现出血,NIHSS评分及病死率明显升高。结论急性脑梗死合并上消化道出血预后不良,早期应采取措施,积极预防。

Risk factors and outcomes of acute cerebral infarction complicated with upper gastrointestinal hemorrhage

Objective To understand the risk factors and outcomes of acute cerebral infarction complicated with upper gastrointestinal hemorrhage.Methods The clinical data of 42 cases of acute cerebral infarction complicated with upper gastrointestinal hemorrhage in the second affiliated hospital of Xuzhou medical college were studied.Results The incidence rate of upper gastrointestinal hemorrhage was related with disturbance of consciousness.The Helicobacter pylori（Hp） infection rate in the acute cerebral infarction complicated with upper gastrointestinal hemorrhage was high.The upper gastrointestinal hemorrhage always happened within 7 days.When out of hospital,the NIHSS and the incidents rate of death was especially higher in patients with acute cerebral infarction complicated with upper gastrointestinal hemorrhage than that in patients non-complicated with upper gastrointestinal hemorrhage.Conclusion The patients with acute cerebral infarction complicated with upper gastrointestinal hemorrhage had a poor prognosis,preventive measures should be made at early stage.
