子宫内膜癌患者ER、PR、C-erbB-2、nm23、Bcl-2检测的临床意义

摘    要:目的研究雌激素受体(ER)、孕激素受体(PR)、C-erbB-2蛋白、nm23蛋白、Bcl-2蛋白在子宫内膜癌中的表达及其意义。方法应用免疫组化S-P法检测30例子宫内膜癌中ER、PR、C-erbB-2、nm23、Bcl-2的表达情况,并以15例癌旁正常内膜组织作对照。结果ER、PR在子宫内膜癌的表达与临床分期及病理分级有关,即临床分期越高,PR表达越弱。ER、PR表达随病理分级增高而增高,即在低分化癌中ER、PR呈低表达或不表达。C-erbB-2、nm23、Bcl-2在子宫内膜癌均有过度表达,nm23阳性表达率随临床分期及病理分级的增高而减低,即在有浸润、转移和低分化癌中无表达。Bcl-2与病理分级有关,即在高分化癌中有较高的表达(92.7%)。结论ER、PR水平的高低对估计预后和临床选择内分泌治疗具有重要意义;nm23基因可能在抑制子宫内膜癌发生转移中发挥重要作用,Bcl-2基因的激活和失控可能在子宫内膜癌发生中具有重要作用,Bcl-2蛋白的检测有助于子宫内膜癌的诊断。

Abstract:Objective To investigate the expression of ER,C-erbB-2, nm23 and Bcl-2 in endometrial carcinoma and their significance.Methods Using immunohistochemical S-P method, ER, PR, C-erbB-2, nm23 and Bcl-2 were deteced in 30 cases of endometrial carcinoma and 15 cases of normal endometrial tissues beside carcinoma.Results The expression of ER and PR was related to clinical stage and histological grade;the expression of ER was decreased with clinical stage progress . ER and PR increased with increasing degree of differentiation of the carcinoma; the expressions of ER and PR were lower or negative. The positive rates of C-erbB-2,nm23 and Bcl-2 were high in carcinoma; the positive rate of nm23 was decreased with increasing pathological grade and clinical stage; the expression of nm23 was negative in low differential carcinoma, with soakage and metastasis. Bcl-2 related with pathological grade;the positive rate was higher(92.7%) in high differential carcinoma.Conclusions The expressions of ER and PR have important significance for prognosis and endosecretory therapy.nm23 gene maybe play an important role to control metastasis, the activation and lose control of Bcl-2 gene maybe was process in occurrence and development in endometrial carcinoma. Bcl-2 protein assay is helpful to diagnosing endometrial carcinoma.
