颈动脉粥样硬化斑块与缺血性脑血管病危险因素相关性多因素分析

摘    要:目的 分析脑梗死危险因素及颈动脉系统内-中膜厚度对脑梗死的影响.方法 155例患者分为脑梗死患者(n=74),非脑梗死患者(n=81).各患者均进行颈动脉血管彩超检查颈动脉系统内-中膜厚度,同时检查胆固醇(TC),甘油三脂(TG),低密度脂蛋白(LDL-C)高密度脂蛋白(HDL-C),纤维蛋白原(Fbg).空腹血糖(FPG),年龄、血压等指标.用Logistic回归数字模型分析,观察指标与缺血性脑卒中相关性研究及相关危险因素分析,以说明上述因素之间以及其与缺血性脑血管病之间的相关性.结果 本研究显示,在L斑块单因素分析中,年龄、收缩压、FBG、L-CCA、L-BIF、L-ICA、R-CCA、R-BIF、R-ICA与脑梗死有明显的正相关(P〈0.05),提示上述因素是缺血性脑血管病的危险因素.R斑块单因素分析中,年龄、舒张压、L-CCA、L-BIF、L-ICA、R-BIF、R-ICA与缺血性脑血管病有明显的正相关(P〈0.05),提示上述因素也是缺血性脑血管病的危险因素.在L-斑块对缺血性脑卒中影响的多因素分析中,年龄、L-BIF与缺血性脑卒中的关系的显著性(P〈0.05),在R-斑块对缺血性脑卒中影响的多因素分析中,R-CCA、R-BIF与缺血性脑卒中的关系的显著性(P〈0.05).结论 颈动脉粥样硬化斑块与缺血性脑卒中密切相关,是缺血性脑卒中的危险因素.

The multivariate logistic regression analysis on arotid atherosclerosis plaque and the risk factors of ischemic cerebrovascular disease

Abstract:Objective To explore the risk factors of cerebral infarction and carotid arteries system intima-media thickness in patients with cerebral infarction. Methods 74 patients with cerebral infarction and 81 patients without cerebral infarction were checked by carotid color doppler ultrasonic, simultaneously blood was measured. The indexes including carotid arteries system intima-media thickness, cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), fibrinogen (Fbg), fasting plasma glucose (FPG), age and blood pressure were recorded and the logistic regression analysis was carried out. Results The univariate analysis on left plaque showed age, systolic blood pressure, Fbg, left common carotid artery (L-CCA), L-BIF, left internal carotid artery (L-ICA), right common carotid artery (R-CCA), R-BIF and right internal carotid artery (R-ICA) were significantly positive correlated with cerebral infarction (P〈 0.05). The univariate analysis on right plaque showed age, diastolic blood pressure, L-CCA, L-BIF, L-ICA, R-BIF and R-ICA were significantly positive correlated with cerebral infarction (P〈 0.05). These factors may be the risk factors of ischemie cerebrovascular disease. The multivariate analysis on left plaque showed age and L-BIF were significantly correlated with cerebral infarction (P〈 0.05). The multivariate analysis on fight plaque showed R-CCA and R-BIF were significantly correlated with cerebral infarction (P 〈 0.05). Conclusion Arotid atheroselerosis plaque is closely related to ischemic stoke and the risk factor of ischemic stoke.
