婴幼儿耳聋的脑干听觉诱发电位特征

摘    要:背景 脑干听觉诱发电位为婴幼儿耳聋的诊断提供客观依据 ,也使早期听力筛选及时分辨外周或脑干的病变成为可能.目的 探讨脑干听觉诱发电位( brain stem auditory evoked potential,BAEP)特征对婴幼儿耳聋早期评估的意义.设计 以诊断为依据的观察性研究.地点和对象 2000- 10/2003- 05惠州市中心人民医院门诊及住院的患者 80例,男 48例,女 32例,年龄 37周~ 4岁,平均 2.5岁.方法 采用丹麦 DANTEC KEYPOTNT 肌电图诱发电位仪对 80例有语言障碍的患儿进行 BAEP检查,并对结果进行分析.主要观察指标 Ⅰ,Ⅱ,Ⅲ,Ⅳ,Ⅴ波潜伏期,Ⅰ~Ⅲ波峰间期,Ⅲ~Ⅴ波峰间期,Ⅲ~Ⅴ,Ⅰ~Ⅲ波峰间期比值.结果 80例 160只耳中,正常耳 30只,异常的 130只耳中周围性通路异常 92耳,蜗后病变 38耳.其中周围性通路异常分别为轻度 12耳、中度 25耳、重度损害 55耳.结论 BAEP检查可以判断早期婴幼儿耳聋听觉通路损害的意义.

Abstract:BACKGROUND:Brain stem auditory evoked potential(BAEP) provides an objective gist for the diagnosis of infant deafness,which also makes early auditory screening to differentiate peripheral or brain stem pathological changes in time become possible. OBJECTIVE:To investigate the significance of BAEP characteristics in the early evaluation of infant deafness. DESIGN:An observatory trial based on diagnosis. SETTING and PARTICIPANTS:Eighty patients from Outpatient and Inpatient Departments of Huizhou Central People's Hospital from October 2000 to May 2003 were selected including 48 male and 32 female cases aged from 37 weeks to 4 years old with a mean age of 2.5 years old. METHODS:Denmark DANTEC KEYPOTNT electromyogram(EMG) evoked potential analyser was used for BAEP test in 80 patients with language impairment and the analysis of the results as well. MAIN OUTCOME MEASURES:I,II,III,IV,and V wave latency,I-III wave crest interval,III-V wave crest interval,and the ratio of the peak intervals of III-V to I-III. RESULTS:In 160 ears of 80 cases,30 ears were normal.In 130 abnormal ears,92 ears were peripheral pathway abnormal and 38 ears had posterior cochlear pathological changes.As for the peripheral pathway abnormal cases,12 ears were mild,25 ears were moderate and 55 ears were severe injuries. CONCLUSION:BAEP examination can judge early infant deafness auditory pathway injury.
