冻融对附睾精子ICSI结局及其形态的影响 |

摘 要: | 目的探讨冻融对PESA—ICSI结局以及附睾精子形态的影响。方法回顾性分析2006年1月之008年12月广州医学院第三附属医院生殖中心因梗阻性无精症或严重少弱精症首次接受PESA—ICSI或ICSI不孕夫妇的治疗结果。共行226周期助孕治疗,包括22个冻融成功的PESA—ICSI周期(研究组),164个新鲜PESA—ICSI周期(对照组)和40个射出精液ICSI周期(研究对照组)。比较三组的受精率、临床妊娠率、种植率和流产率。对33例患者冻存的PESA精子冻融前后均进行形态学观察,自身对照比较正常精子百分率、头部异常百分率、颈及中段异常百分率、尾部异常百分率及TZI、SDI的变化。结果研究组、对照组及空白对照组三组患者的一般情况无统计学差别(P〉0.05),三组受精率、临床率、种植率及流产率相比亦无统计学差别(P〉0.05)。PESA精子冻融后TZI较冻融前明显降低,1.43±0.12vs2.36±0.08(P〈0.01),其余各项指标相比无差异。结论冻融未影响附睾精子ICSI妊娠结局,冻融过程可以淘汰部份畸形精子。

Abstract: | Objective To investigate the effect of freezing-thawing process on the outcome of epididymal spermatozoa-ICSI and the morphology of epididymal spermatozoa. Methods The outcome of ART were analyzed in infertile males with obstructive azoospermia and severe oligospermia and asthenozoospermia retrospectively from 2006 Jan to 2008 Dec in our ART centre.There are 226 cycles in total including 22 cycles of frozen-thawed PESA-ICSI(study group), 164 cycles of fresh PESA-ICSI(control group) and 40 cycles of ejaculated semen ICSI(blank control group ).Fertilization rate,clinical pregnancy rate, implantation rate and abortion rate were compared among the three groups.The morphology of frozen and thawed PESA spermatozoa in 33 patients was observed. Results Thetr no significant difference were observed in the three groups in fertilization rate,clinical rate,implantation rate and abortion rate. (P>0.05). TZI of epididymal spermatozoa after thawed was significant decreased, 1.43±0.11 vs 1.36±0.08(P<0.01)respectively. Conclusions Freezing-thawing process does not affect the outcome of epididymal spermatozoa-ICSI and this process can also get rid of some deformity sperm. |
