脑内海绵状血管瘤的MRI表现及弥散成像的诊断价值

摘    要:目的 探讨脑内海绵状血管瘤的MRI表现及磁共振弥散成像(DWI)的诊断价值。方法 回顾性分析45例经病理证实的脑内海绵状血管瘤患者的MRI资料,重点分析DWI信号特征及其对病灶的显示能力。结果 45例共捡出47个病灶,34个病灶T,WI和T1WI均呈网状高、低混合信号,8个病灶T,WI1T,WI均呈高信号,4个病灶T1WI、T1WI均呈低信号,1个病灶T1WI呈等信号、T1WI呈稍低信号,46个病灶(占97.87%)T1WI灶周具有“铁环征”:DWI显示的病灶范围包括瘤体及灶周舍铁血黄素区域,敏感性较高、可提高病灶的显示率。结论 MRI是诊断脑内海绵状血管瘤的首选方法,常规SE序列与弥散成像相结合,能更准确、全面地提供诊断信息。

Abstract:Objective To evaluate MRI features of cerebral cavernous angioma and discuss the value of diffusion weighted magnetic resonance imaging(DWI) Methods MRI findings of 45 cases with cerebral cavernous angioma,proved by pathology,were reviewed. The signal characteristics of DWI and its ability to demonstrate lesions were studied emphatically. Results In 45 patients,there were 47 lesions,of which 34 lesions were mesh mixture of hyperintensity or hyopintensity on T1WI and T2WI,8 lesions hyperintensity on T1WI and T2WI respectively,4 lesions isointensity on T1WI and T2WI,and 1 lesions was isointensity on T1WI and hypointensity on T2WI. 46 lesions(97.87%) around had black hypointensity ring on T2WI.DWI showed nidus and haemosiderin around the lesions and had higher sensitivity,which could improve the detection rates of the lesions. Conclusion MRI,the first choice for the diagnosis of cerebral cavernous angioma,can accurately provide information combining with routine SE sequence and DWI.
