胰管内置管外引流在胰十二指肠切除中的应用

目的 介绍胰头十二指肠切除术中胰管内置管外引流的方法和体会。方法 在空肠和胰腺吻合前，于主胰管内置放导管，在空肠内穿行约30厘米后穿出空肠引出体外，将胰液外引流。结果 7例患者均未发生胰瘘。结论 胰管内置管将胰液外引流，一方面减少了吻合口处肠内容物，同时也降低了该处肠管内的压力；另一方面将胰液引出体外，避免了胰酶对吻合口的自身消化,吸效的防止了吻合口瘘。

Application of external drainage by inserting catheter into pancreatic duct in pancreatoduodenectomy

Objective To introduce the method and experience of external drainage by inserting catheter into pancreatic duct in pancrea toduodenectomy (PD). Methods A catheter was put into the main pancreatic duct before the jejunum was anastomosed with the pancreas. The catheter was drained outside the jejunum and body wall after remaining inside the jejunum for about 30cm for draining the pancreatic juice externally. Results No pancreatic fistula was found in 7 cases after this operation. Conclusion Drainage of pancreatic juice externally by putting catheter in the pancreatic duct may reduce the intestinal content and simultaneously decrease the inner pressure of the site of anastomosis. It may prevent the occurrence of fistula at the anastomotic site by avoiding the digestive effect of pancreatin at the site of anastomosis.
