乳酸脱氢酶同工酶分析在心脏病诊疗中的探讨

摘    要:目的 比较分析乳酸脱氢酶(LDH)同工酶1与2的百分比在心肌梗死、心肌炎、不稳定型心绞痛型冠心病与非心脏疾病中的差别,从而阐述LDH同工酶1与2的检测在心脏病诊断中的临床意义。方法 用常规的琼脂糖电泳法。组间差异的统计分析方法用方差分析。结果 LDH1百分比在心肌梗死型冠心病组与心肌炎组、不稳定型心绞痛型冠心病组和非心脏病对照组间具有显著性差别(P<0.05,F检验)。而它在心肌炎组、不稳定型心绞痛型冠心病组和非心脏病对照组间无显著性差别(P>0.2,F检验)。而它在心肌炎组、不稳定型心绞痛型冠心病组和非心脏病对照组间则无显著性差别(P>0.2,F检验)。LDH2的百分比在4个实验组中无显著性差别(P>0.2,F检验)。结论 从实验结果的统计分析和8例心脏手术病人LDH1动态性检测结果表明,对临床上有心肌损伤的心脏病,LDH1可作为监测疗效、判断预后的生化指标。

Abstract:Objectives To clarify the clinical significance of the examination of LD 1 and LD 2 on heart disease diagnosis and therapy by comparing and analyzing the difference of the percentage of LD 1 and LD 2 among myocardial infraction group,myocarditis group.the unstable angina pectoris coronary heart disease group and non-heart disease group.Methods The percentage of LDH 1 and LDH 2 of each sample was examined with routine agarose gel electrophoresis and then the result was analyzed with analysis of variance.Results It is show that there is statistical difference significance of LDH 1 percentage between myocardial infraction group and unstable angina pectoris coronary heart disease group,myocarditis group,non-heart disease group (P<0.05,F test). But there is no statistical difference on LD 1 percentage among unstable angina pectoris coronary heart disease group,myocarditis group and non-heart disease group (P>0.2,F test). The LD 2 percentage among four groups not show any difference significance(P>0.2,F test). Conclusions From the statistic analysis of numerous experiments and the LD 1 percentage dynamic examination of eight surgery cases,it is shown that the LD 1 percentage can be used as a useful biochemical sign for evaluating the treatment effect and judging the prognosis of heart disease with myocardiolysis.
