基础泪液分泌试验Ⅰ泪膜破裂时间检测的影响因素

摘    要:目的 研究基础泪液分泌试验Ⅰ、泪膜破裂时间检测的影响因素,探讨干眼的临床诊断策略.方法 对125例疑诊干眼患者在睑裂开放状态(睁眼)下和睑裂闭合状态(闭眼)下行基础泪液分泌试验Ⅰ(Schirmer Ⅰ test,Sit);对80例疑诊干眼患者分别进行0.125%和0.25%荧光素钠(fluorescent,fl)测试泪膜破裂时间(Break-up Time,BUT),对上述结果进行对照分析.结果 闭眼状态下的Sit(13.2±9.1)mm小于睁眼状态下的Sit(16.2±9.7)mm,其差异有显著性(P<0.05).对相同患者0.125%fl测试的BUT(10.8±7.4)s大干0.25%fl测试的BUT(6.21±5.70)s,二者差异有极显著性(P<0.001).对不同受试者0.125%fl测试的BUT(10.8±7.4)s大于0.25%fl测试的BUT(8.4±6.9)s,二者差异有显著性(P<0.05).结论 Sit、BUT检查的影响因素众多,应当制定统一、规范化的操作程序,以尽可能减少操作所带来的误差.建议在睁眼状态下测试Sit;用0.25%或更高浓度的fl或荧光素试纸条进行眼表染色,用以测试BUT.

Abstract:Objective To learn the factors affecting Schirmer Ⅰ tests and tear break-up time of dry eye.Discnss the clinical tactics to diagnose dry eye.Methods Compared and contrasted the Sit (Schirmer Ⅰ test)value between open-eye group and close-eye group in 125 patients.Compared and contrasted the BUT (tear break-up time)value of 80 patients who accepted the examination with 0.125% fluorescent stain and 0.25% fluorescent stain orderly.Results Sit (13.2±9.1)mm measured in dose-eye group was less than Sit (16.2±9.7)mm measured in open-eye group (P<0.05).In the same patient, BUT (10.8±7.4)s assessed by 0.125% fl first was more than BUT(6.21±5.70)s assessed by 0.25% fl later(P <0.001).In different patients, BUT(10.8±7.4)s assessed by 0.125% fl was more than BUT (8.4±6.9)s assessed by 0.25% fl (P<0.05).Conclusions There are many factors affecting SIt and BUT.We should unify and standardize the operating rules of the examinations of dry eye to reduce error led by operation.
