细胞角蛋白19和人乳腺球蛋白在乳腺癌阴性前哨淋巴结中的表达及临床意义

摘    要:目的研究乳腺癌常规病理检查阴性的前哨淋巴结中细胞角蛋白19(cytokerantin-19,CK19)和人乳腺球蛋白(human mammaglobin,hMAM)的表达及临床意义。方法采用免疫组化SP法检测60例乳腺癌阴性前哨淋巴结中CK19和hMAM的表达,并分析两者表达差异及与临床病理因素的关系。结果 60例乳腺癌阴性前哨淋巴结经免疫组化SP法检测hMAM表达2例,阳性检出率为3.3%。CK19除了hMAM阳性2例外,另外检出6例,阳性检出率为13.3%。CK19表达与肿瘤T分期有关(P<0.05),与患者的年龄、月经状况、定位方法、肿瘤位置、病理类型、雌激素受体表达、孕激素受体表达、CerbB-2表达、Ki-67表达和肿瘤分子分型均无相关(P>0.05);hMAM表达与以上临床病理因素均无相关(P>0.05)。结论 CK19和hMAM均可作为检测乳腺癌阴性前哨淋巴结微转移的指标,但CK19是检测乳腺癌阴性前哨淋巴结微转移的敏感指标。

Abstract:Objective To research the expression and clinical significance of CK19 and hMAM in negative sentinel lymph node of breast cancer. Methods The expression of CK19 and hMAM in negative sentinel lymph node of 60 patients with breast cancer were detected with SP immunohistochemical method. The difference between two groups and the relationship between CK19 and hMAM expression and the clinical pathological factors were analysed. Results The positive rates of CK19 and hMAM expression in negative sentinel lymph node of breast cancer were 13.3% and 3.3% respectively. There were no association between CK19 expression in negative sentinel lymph node and the age of breast cancer patients, menstrual status,positioning methods, tumor location, pathological category,estrogen receptor expression, progesterone receptor expression, CerbB-2 expression, Ki-67 proliferation expression and tumor molecular classification (P>0.05).But there was dependability between the CK19 expression and tumour size (P<0.05). There were no association between the hMAM expression and all above clinical pathological factors (P>0.05). Conclusions The CK19 expression is a sensitive micrometastasis indicator in negative sentinel lymph node of breast cancer.
