食管恶性纤维组织细胞瘤临床病理及文献复习

目的观察食管恶性纤维组织细胞瘤的临床病理形态学特征、诊断、鉴别诊断及免疫组化特点。方法对1例食管恶性纤维组织细胞瘤的临床表现、大体及光镜形态及免疫组化结果进行观察,并复习有关文献。结果食管恶性纤维组织细胞瘤均发生于中老年男性,并且位于食管的中下段,多呈息肉样或蕈伞样突入食管腔,细胞成分较多,可见异型梭形的纤维细胞,圆形、卵圆形的组织细胞,单核及多核的巨细胞,少量的炎细胞,核分裂较多见。免疫组化：Vimentin（＋）、CD6（8＋）、SMA（-）、S-100（-）、CD11（7-）、CD3（4-）、CK（-）、EMA（-）、HMB45（-）。结论食管恶性纤维组织细胞瘤是一种罕见的食管恶性肿瘤,发生机制目前尚不清楚,其诊断依赖于病理检查及免疫组化。

Clinicopathological observation and literature review of malignant fibrous histocytoma of esophagus

Objective To observe characteristics of pathology morphology, diagnosis and antidiastole of malignant fibrous histocytoma of esophagus, and the characteristics of immunohistochemistry. Methods Clinical manifestation, light microscope, and immunohistochemisal examination was analyzed in one patient with malignant fibrous histocytoma of esophagus, and relative literature was reviewed. Results Malignant fibrous histocytoma of esophagus was not rare in middle and old aged male and in the middle and lower part of esophagus, with the shape of polypus or mushroom. There were many cell components, from where heterogenic and fusiform fibroblast,round and ovate rhagiocrine cells; mononuclear and multinuclear giant-cell , as well as a spot of inflammatory corpuscle could be observed. In addition, caryocinesia can be seen more.Immunohistocbemistry ： Vimcntin（ ＋ ）CD68（＋） SMA（ - ） S-100（- ） CDll7（ - ） CD34（ - ） CK （-） EMA （-） HMB45 （-）.Conclusion Malignant fibrous histocytoma of esophagus was rare,we could not tell its etiapathogencsis at present.And definite diagnosis depends on pahhology and immunohistochemistry.
