老年慢性阻塞性肺疾病患者抑郁相关因素调查分析

目的探讨老年慢性阻塞性肺疾病（chronic obstructive pulmonary disease，COPD）稳定期患者抑郁发生率及其相关因素，为临床综合诊治提供相应的指导。方法选取天津医科大学总医院呼吸科就诊的老年COPD稳定期患者82例。通过老年抑郁量表（GDS）及社会支持度量表以问卷形式进行调查。应用SPSS 11．5统计软件进行多元逐步回归分析。结果老年COPD稳定期患者抑郁的发生率是54％，按GDS分数分组与调查结果进行方差分析得出经济收入、婚姻、文化程度、戒烟年限、合并症、喘息年限、社会支持总分数、主观支持分数、支持利用度分数差异有统计学意义，多元回归后进入方程的为社会支持总分数（β=0．377，P〈0．005）、文化程度（β=0．328．P〈0．05）、婚姻状况（β=-0．308,P〈0．05）与抑郁相关。结论老年COPD患者抑郁发生率较高并与患者的社会支持度、文化程度、婚姻状况因素相关，应该在治疗躯体疾病的同时注意心理疏导和干预。

Analysis on related factors of depression in elderly patients with chronic obstructive pulmonary disease

Objective To investigate the occurrence of depression and related factors in elderly patients with chronic obstructive pulmonary disease(COPD)at stable stage.Methods Eighty-two COPD at stable stage cases were selected to investigated and analyzed with Geriatric Depression Scale(GDS) and Social Support Rating Scale.Application of SPSS statistical software for multiple stepwise regression analysis.Results The depression of elderly patients with COPD at stable is 54%.GDS scores grouped according to the survey results with analysis of variance derived income,marriage,education,smoking cessation age,complications,breathing life,social support total score,subjective support scores,support the use of degrees and depression scores were statistically significance.Social support (β= 0.377,P＜0.008),Education(β= 0.328,P＜0.05),Marital status (β=-0.308,P＜0.05) is the main factors of influence depression.Conclusions The depression of elderly patients with COPD at stable stage is higher and correlated with many factors,such as patient's social support,education,marital status.It should be to divert the attention and intervention as well in treatment.
