向急性白血病转归的骨髓增生异常综合征临床与形态学研究

目的]对转归急性白血病的骨髓增生异常综合征患者的临床与形态学研究。方法]78例MDS患者做一般资料分析,对22例转归AL患者的MDS阶段及MDS转归白血病临床及血液学特征,进行回顾性分析。结果]78例MDS患者中22例转归为急性白血病,占28.2%,其中20例转为急性非淋巴细胞性白血病(ANLL),2例转化为急性淋巴细胞性白血病(ALL),转归时间随MDS分型不同而长短不一,且转归白血病时间长短与血及骨髓中原始细胞数目有相关性,血液学特征主要表现为外周血一系二系或三系细胞减少,粒、红、巨三系血细胞的病态造血伴不同程度的临床症状,12例接受染色体检测患者中4例有异常核型的改变。结论]对转归白血病的骨髓增生异常综合征的临床与形态学研究有助于对MDS的诊断分型研究。

Objective] Clinical and morphology study on myelodysplastic syndrome (MDS) changing into leukemia.Methods] Seventy-eight cases of general information analysis of MDS patients,and make review analysis on the clinical and hematology of 22 cases of patients changing into AL from MDS as well as MDS changing into leukemia.Results] Seventy-eight cases of MDS patients changing into leukemia,accounting for 28.2 %,of which 20 cases changed into ANLL,2 cases changed into ALL,the time of changing is various according to the different MDS types,and the duration of changing into leukemia has something with the original cell number in blood and marrow,whose hematology characteristics appear as the decrease of one system,two system or three system of circumferential blood,and the clinical syndrome of grain,red,and huge three system blood cell morbidity.There are four cases abnormal karyon change in the 12 cases receiving chromosome examination.Conclusion] It is helpful for the diagnosis and typing of MDS to study the clinical and morphology on myelodysplastic syndrome(MDS) changing into leukemia.
