小瞳孔角膜缘隧道切口人工晶状体植入术

目的 探讨小瞳孔状态下经角膜缘隧道切口人工晶状体植入术的技巧并评价疗其疗效。方法 经比颞角膜缘隧道切口，进行虹膜后粘连分离， 瞳孔缘剪开扩张瞳孔及碎核技术，对26例（28眼）并发性、外伤性和青光眼术后白内障施行白内障摘出人工晶状体植入术。结果 术后裸眼视力≥0．3者20眼（71．4％）。9眼应用非切开瞳孔扩张法，术后全部病例瞳孔恢复原状；19例行瞳孔缘切开扩大瞳孔，术后13眼基本恢复圆瞳孔，6眼瞳孔欠规整。结论 白内障全虹膜后粘连的小瞳孔，通过行虹膜后粘连分离瞳孔扩张成形术后，仍可经角膜缘隧道切口摘出白内障植入后房人工晶状体并基本恢复生理性圆瞳孔。

Posterior chamber intraocular lens implantation with small pupil through a limbus tunnel incision

Objective To investigate and evaluate the surgical techniques and therapeutic effects of posterior chamber intraocular lens (IOL) implantation in cases with small pupil through a limbus tunnel incision.Methods The surgery was performed by using posterior synechialysis,cuting pupillarry margin to dilatate the pupil and fragmentating the lens nucleus through a temporal libus tunnel incision on 28 eyes of 26 cases of complicated,traumatic and glaucoma postoperative cataract with small pupil.Results The postoperative uncorrected visual aucity in 20 eyes (71.4%) was 0.3 or better.In 9 eyes,the lens were extracted by pupil dilatating without the pupillary margin cuting.After the surgery all the 9 eyes had the pupils recovered to normal shape and size.In 19 eyes,the procedure was performed by cuting pupillary margin,after the surgery,a round pupil was obtained in 13 cases and an irregular in 6 cases .Conclusions The posterior chamber IOL can be primarily implanted in cases with posterior synechia and contracted pupil through a limbus tunnel incision after synechialysis and pupil dilatating method,the pupil can be general recovered to normal.
