盆腔内脏神经的解剖学研究及临床应用

摘    要:目的:为防止和减少直肠癌扩大根治术引起的排尿和性功能障碍提供神经解剖学资料。方法:解剖并测量20例成人男尸的盆丛,骶2-4盆内脏神经膀胱丛,前列腺丛和直肠丛的神经及其分支。结果:得出盆腔各神经丛的分支数量、长度、直径及各种神经相互吻合关系。结论:保留骶2-4神经袢和分支,对减少术后功能障碍有益;盆丛的易损伤区在直肠中段和膀胱侧壁。

Abstract:The purpose of this study was to provide neuroanatomical data for preventing and decreasing problems in urination and sexual dysfunction which were caused by expansion any radical operation in rectum cancer. Methods:We measured the nerve and nervous branches in the pelvic plexus, the pelvicovisceral nerve of Sacral 2 4 nerve plexus of urinary bladder, the nerve plexus of prostate gland and the rectal nerve plexus in twenty adult cadavers.Results:anastomosis were observed.Conclusions:The pelvicovisceral nerve should be prevented from damaging during operation except for cancer cells invading these nerves. The lymphatic nodes should be eliminated in the radial cure operation. Reservation of the Sacral 2 4 nerve loops and their branches was very beneficial for reducing the postoperative dysfunction.The easier damaging area of pelvic plexus nerve is in the middle segment of the rectum and the lateral wall of the urinary bladder.
