64排CT回顾性心电门控成像对新生儿复杂性先心病的诊断价值

摘    要:目的:探讨64排螺旋CT回顾性心电门控成像对新生儿复杂性先天性心脏病(CHD)的诊断价值。方法:210例新生儿复杂性先天性心脏病患儿分别采用64排螺旋CT回顾性心电门控对比增强心脏成像扫描和普通对比增强心脏成像扫描方案,评价两组的图像质量和诊断结果。结果:研究组可评估和不可评估的图像为100.0%(114/114)、0%(0/114),对照组分别为97.9%(94/96)、2.1%(2/96),可评估图像两组的差异无统计学意义(P>0.05),而对于不可评估的图像两组差异有统计学意义(P<0.05)。研究组复杂性先心病诊断准确率和冠状动脉异常准确率分别为97.4%(111/114)和100.0%(6/6),对照组分别是94.8%(91/96)和50.0%(2/4),两组对复杂性先天性心脏病诊断准确率比较差异无统计学意义(P>0.05),但对冠状动脉异常准确率差异有统计学意义(P<0.05)。结论:64排螺旋CT回顾性心电门控成像可以提高新生儿复杂性先天性心脏病图像质量,提高新生儿复杂性先天性心脏病诊断准确率,尤其是提高伴有冠状动脉异常诊断的准确率。

Abstract:Objective:To explore the value of 64-detector CT with retrospective electrocardiographic gated technique for diagnosis of neonates with complexed congenital heart disease(CHD).Methods:Two hundred and ten neonates with complexed CHD adopted 64-detector CT with retrospective electrocardiographic gated technique for contrast enhancement cardiac scan and routine contrast enhancement cardiac scan,respectively,then the image qualities and diagnostic results were evaluated in the two groups.Results:The assessable and unassessable images in study group accounted for 100.0%(114/114) and 0%(0/114),respectively;the assessable and unassessable images in control group accounted for 97.9%(94/96) and 2.1 %(2/96),respectively,there was no statistically significant difference in the assessable images between the two groups(P>0.05),but there was statistically significant difference in the unassessable images between the two groups(P<0.05).The diagnostic accuracy rates of complexed CHD and coronary artery anomalies in study group were 97.4%(111/114) and 100.0%(6/6),respectively;the diagnostic accuracy rates of complexed CHD and coronary artery anomalies in control group were 94.8%(91/96) and 50.0%(2/4),respectively,there was no statistically significant difference in the diagnostic accuracy rate of complexed CHD between the two groups(P>0.05),but there was statistically significant difference in the diagnostic accuracy rate of coronary artery anomalies between the two groups(P<0.05).Conclusion:Retrospective electrocardiographic gated technique of 64-detector CT can improve the image quality and the diagnostic accuracy rate of neonates with complexed CHD,especially the diagnostic accuracy rate of complexed CHD combined with coronary artery anomalies.
